Physician - Full Time, Part Time & Per Visit
Hospice Care Network, a member of the North Shore-LIJ Health System, is a JCAHO accredited, not-for-profit healthcare organization. HCN currently seeks an enthusiastic physician to practice end-of-life care in a high quality, collegial and professionally stimulating environment. Join a team of physicians and other professionals to direct patient care, conduct home visits, and provide consultative services. Our integrated electronic clinical records system assures superior access to clinical information across the continuum of care. You will participate in Interdisciplinary Team Meetings, complete assigned administrative duties, and have the opportunity to teach palliative medicine to staff, community physicians, and medical students.
Board Certification in hospice and palliative care and/or fellowship training is desired. Board Certification in internal medicine, family practice or other medical specialty is required, as well as excellent collaboration, communication and primary care medical skills. Bilingual is a +.

Hospice Aide - Suffolk, Nassau & Queens Counties
You can make a difference! You can make a difference in the lives of both the patient and those they care about by becoming part of Hospice Care Network's patient care team. Minimum current hourly rate is $12.25 + mileage between patient homes.
You must have:
- A certificate showing you are a New York State certified Home Health Aide (HHA).
- A current valid driver's license and ability to provide own transportation to patient residences.
- A minimum one-year experience in homecare or nursing home setting preferred.
- Scored 75% or higher on HCN's skills assessment test.
You can make a difference by following the patient's plan of care established by an HCN registered nurse and other members of the team. This plan of care may include tending to a patient's personal hygiene, assisting with feeding or ambulation, performing light housekeeping tasks essential to the patient's health, etc. You will also be required to document in writing the care you provide as well as communicate effectively with the patient/caregiver and members of the HCN patient care team.

Registered Nurse - Long Island & Queens
Full Time, Per Visit - Days, Evenings, Nights & Weekends
Discover a fulfilling and often uplifting professional experience working with our expanding Hospice Care Network Team. You will focus on caring for, not curing, patients with advanced illness, and helping to make Every Day Count. The care and compassion you provide for the patients, and the support you provide for the family and caregivers, will enable the patients to live in peace and with dignity.
Working in Queens, Nassau or Suffolk County, you will assess patients and implement the nursing portion of the care plan, as well as teach, supervise, and counsel patients, family members and other caregivers regarding physical care and patient's terminal condition. You will also provide supervision to LPN's and HA's on the hospice team.
Qualifications include:
- NY State Licensed RN with 2 years recent bedside or medical/surgical nursing experience, ideally with homecare, hospice or oncology patients.
- Valid driver's license and own vehicle.
- Candidates must also have good communication and listening skills.
- PC literacy and a courteous and professional manner.
- Ability to communicate effectively in Spanish, Chinese, or Korean for Queens County is a plus
