HCN also provides inpatient hospice care for hospice patients with acute symptoms at a dedicated 7 room unit at Franklin Hospital, 3 specially decorated rooms at Forest Hills Hospital and up to 6 rooms in the Palliative Care Unit at North Shore Hospital at Manhasset.
Franklin Hospital
Franklin Hospital is a member of North Shore-LIJ Health System and a 305-bed community hospital offering a comprehensive array of programs and services to patients from Nassau and Southeastern Queens Counties. The Hospice & Palliative Care Center at Franklin services people in the final stages of life, as well as their families, with care and comfort in an inpatient setting designed to feel like home. Private rooms, homelike décor and parlors for family respite contribute to a peaceful, dignified environment. People and their families are treated with the utmost compassion by a dedicated and responsive staff, and can take comfort in unlimited visitation and sleep-in accommodations.

Forest Hills Hospital
Forest Hills Hospital is a member of North Shore-LIJ Health System. As a 222-bed community hospital located in an ethnically diverse residential neighborhood in northern Queens, it is their mission to provide the highest quality and most compassionate healthcare in the county, with sensitivity and respect for the cultural needs of people and their families. The goal of their hospice unit is to stabilize acute symptoms and plan for a continuation of care with hospice at home or at a hospice unit in the nursing home. People entering the new hospice unit are officially discharged from the hospital. The unit includes two renovated rooms, a family meeting room and a waiting area.

The Hospice Suites at Harborside
Hospice Care Network has collaborated with the Amsterdam at Harborside to provide the highest level of care to those with advanced illness. The Amsterdam at Harborside is Nassau County’s first and only true life care community, offering independent senior living and a continuum of health care services. These services are offered at the Tuttle Center which includes enriched housing, memory support, skilled nursing and rehabilitation, hospice and palliative care. At The Hospice Suites at Harbor Side you can expect the highest level of care. The Amsterdam is located on the North Shore of Nassau County in the community of Port Washington.

Directions
From the East:
On the Long Island Expressway, take Exit 39 (Glen Cove Road) and from the exit ramp continue west on the service road for three lights.
Turn right at the third light (Roslyn Road) and continue north until it ends. (The Roslyn Clock Tower will be on the right).
Turn left at the traffic light, stay in the right lane and go under the Via Duct/Overpass. This will become West Shore Road.
You will continue for approx. 1.5 miles.
Make a left onto Fairway Drive and the Amsterdam is 1500 yards up to the right.
From the west:
On the Long Island Expressway, take Exit 37 (Willis Avenue) and from the exit ramp continue on the service road. Turn left at the second light (Roslyn Road) until it ends.
(The Roslyn Clock Tower will be on the right).
Turn left at the traffic light, stay in the right lane and go under the Via Duct/Overpass. This will become West Shore Road.
You will continue for approx. 1.5 miles.
Make a left onto Fairway Drive and the Amsterdam is 1500 yards up to the right.
