The goal of Hospice Care Network is to provide in-home hospice care wherever people live. We work with great effort to address their needs and make sure they live peacefully during the final stages of their lives. With our Hospice Care Network, we can make every day count with your loved ones.

Hospice Care Network strives to help you make the most of your final days. In line with our goals of providing quality care, our team makes efforts to develop personalized support programs. We take time assessing all aspects of your situation, from physical condition, emotional state, to spiritual well-being.
Professional Assistance
We take pride in having a team of professional nurses and caregivers who are always ready to help you live to the fullest until the end. Our team is highly trained in providing hands-on care, from providing medication to managing your daily activities. Hospice Care Network also has physicians who can work with other specialists to develop the most appropriate treatment program. We can also send social workers who can help manage finances or provide additional support to your family.
While the majority of our people prefer to stay in their own homes, we also care for them in designated nursing homes, in assisted living facilities, at The Amsterdam’s Hospice Suites at Harborside and at our own Hospice Inn, a warm home-like hospice residence.

We Protect Patient Privacy In Every Way!
HIPAA - Health Insurance Portability and Accountability Act (1996) creates national standards to protect individuals’ medical records and other personal health information. This law was enacted to protect patient privacy and improve the security, accuracy and integrity of the electronic transmission of health information. HIPAA provisions affect providers (that means Hospice Care Network), payers, and all other entities that share in the exchange of health information. This new law requires that a privacy notice be given to all patients and makes it illegal to release health information to inappropriate parties, or fail to adequately protect health information from release. If violated, civil and criminal penalties can be incurred.
